Personalised Support Planning

When a resident joins Brightpath Housing Limited CIC, a dedicated Support Worker carries out an initial risk assessment to understand the resident’s circumstances and support needs.

A personalised support plan is then developed which sets short, medium and long-term goals designed to support the resident’s progression towards independent living.

Support plans are reviewed at least monthly to ensure progress is maintained and to identify any additional support requirements.

Programme Overview

The Steps to Independence programme forms part of Brightpaths Housing Limited CIC's structured housing support model. It is designed to help residents develop the practical skills, knowledge and confidence required to sustain their own tenancy and live independently.

Residents work through a series of support modules delivered by dedicated Support Workers, focusing on tenancy sustainment, life skills development and long-term stability.

Support is delivered primarily face-to-face within the accommodation and through ongoing engagement with residents throughout their stay.

As residents progress through the programme and develop greater independence, support workers assist them in preparing for their move into independent accommodation.

Support Delivery

Up to 13 hours per month

Residents receive up to 13 hours of structured support per month, delivered by experienced Support Workers.

Face-to-face support

Support is delivered primarily face-to-face and focuses on helping residents develop the skills needed to manage their home, sustain their tenancy and live independently.
